I am a friend or relative of a current/past resident

My husband was admitted to Park Senior Villas, Memory Care, Goodyear, AZ on 9/16/2020. I had several conversations with the Director of Community Relations who advised that the unit my husband was going into was well staffed with a Med Tech and caretakers who had to take an Alzheimer\'s course to famliarize themselves with some of the behaviors of Alzheimer\'s patients. She said there were always several people on the floor at any given time and that even she and other office personnel visited the residents from time to time and were on a first name basis with some of them. My husband and I were interviewed by their Licensed Practical Nurse. She did cognition tests with my husband and I talked with her at length about his medical history and medications he was currently taking. I put this information in writing and emailed the Director of Community Relations a copy. In addition, records from my husband\'s primary care physicians, Internist, Neurologist and Urologist, were requested by the Director and she advised they were received. We also had a conversation about Hospice care and I was told that many of their residents used Sage Hospice and that they were very happy with them. She said hospice was covered by Medicare and that many residents stay on hospice for years, as I did not feel my husband was at the end of his life. I contacted them, a nurse visited my husband and I at our home, and ultimately we signed on with them. I was told that initially a registered nurse would visit my husband at the facility twice a week. I was also reassured by the Director of Community Relations and the LPN that if anything at all changed with my husband that they would call me immediately. Feeling comfortable that my husband was going to get the care and attention he needed, I completed the paperwork for his admission and gave them a deposit. At the time of his admission on 9/16, his medical condition was stable; his blood pressure was within normal limits and he was free of a urinary tract infection, which is common with catheter users. His urologist had put him on a maintenance dose of an antibiotic in July, 2020, and he saw him a few days before he was admitted. The Director of Community Relations advised that because of Covid 19, an appointment needed to be scheduled to visit loved ones, and she also said they strongly advised that there be no visitors for the first two weeks to allow the resident time to acclimate to the facility which made it easier for them. I scheduled an appointment for 9/30/2020. I was told I could call the caretaker line when I wanted to which I did and was always told he was \"fine\", \"O.K.\", \"he ate a good breakfast\", \"going to his room with an ice cream sandwich.\" Feeling all was going well, I looked foward to my phone conversation with the Hospice Nurse, and to my visit on 9/30/2020. However, on 9/29, I received a call from the hospice nurse who said my husband was O.K. but that he was sleeping and moved his arm when she attempted to take his blood pressure so she didn\'t want to bother him and would take it on her visit later in the week. She also said he had a cough but was O.K.\"; that the area around his catheter was \"a little red\" and that she would order an antibiotic ointment, and that she was told he didnt have much of an appetite for a couple of days. Again she reiterated he was O.K. A couple of hours later, I received a call from the facility asking if I had a Do Not Resusitate on my husband. I was what was wrong and was told his blood pressure was low. I told the caller to immediately call 911 to take him to the closest hospital. My daughter and I got into the car and met rescue as they were pulling into the Emergency Room. A little later that day when I was allowed to visit my husband he looked very thin, pale, hadn\'t been shaven in days, his teeth were very dirty, he was wearing the same shirt he had on when he was admitted on 9/16, he had the same shirt on on 9/18, and 9/23 from pictures I was sent from the facility. The shirt was stained and filthy. The doctor spoke with me and said he was very sick. He said he had pneumonia, was severly dehydrated, that his blood pressure was very low and that he was very weak. He said he was doing tests and another doctor would see me with the test results. When I spoke with the doctor about my husbands test results, he said his kidney function tests were so high they were \"off the charts\". He said they were treating him with antibiotics and fluids and would do everything they could to make him better. After 4 days in the hospital, I was told my husband was not going to get better. On 10/3, my husband was brought home via rescue with another hospice group. He died on 10/4/20. The hospital sent my husband\'s medication home with him. The last time he was given his medication at Park Senior Villas was 9/25. I\'m grieving over the loss of my husband, and over his premature death because of the lack of care and neglect he received at Park Senior Villas.

I am a friend or relative of a current/past resident

On the surface Park Senior Villas looks great but in reality it is a mess. In the 4 months my mom was there she received 3 showers. They med techs failed to administer her medication on so may occasions I lost count. The residence were left unattended in the evenings with no care givers in the home. On two occasions there was an entire day where there was not even one care giver in the home. We had multiple meetings and were told things were going to get better, we even had a meeting with the owner and no changes were made. Things actually got worse. The police and fire department had to be called on multiple occasions due to no staff being on site and family members not being able to get into the home after a resident fell and was laying on the floor for over 2 hours when there was no care takers in the home. With the cost being $5000.00 a month just for room and board all we wanted was basic human care. Run away from this place. I would not recommend placing any family members here. The list goes on and on with the lack of care we received at Park Senior Villas.

Financial Assistance for Assisted Living in Goodyear

Medicaid in Arizona

The Arizona Health Care Cost Containment System is the state’s Medicaid system. It provides essential health care to low-income families and individuals in Arizona including pregnant women, disabled persons and senior citizens. Some of the service the AHCCCS covers include doctor visits, dental services, hospitalization, prescription medications and diagnostic tests. 

To qualify for AHCCCS standard coverage as a senior citizen, applicants must be 65 years of age and meet the following financial requirements:

  • Single applicants are limited to a maximum monthly income of $1,041
  • Married applicants are limited to a maximum monthly income of $1,409


There are no asset limitations for the AHCCCS state plan. 

To apply for coverage, seniors living in Goodyear can complete their application online or contact AHCCCS at (800) 565-5465. 

The Arizona Long Term Care System provides coverage for seniors eligible for AHCCCS who require long-term care such as assisted living. This program covers the cost of care services, as well as medical and dental care, prescription medications and specialty care. Approved applicants are assigned a case manager who coordinates their services. 

To qualify for the ALTCS program, applicants must be age 65 or older and require a nursing facility level of care. They also must meet the following financial requirements: 

  • Income below $2,313 per month
  • Countable assets valued at no more than $2,000


Exemptions to the asset limit include the applicant’s home as long as either the applicant or their spouse continues to reside there, as well as a single vehicle and personal belongings. 

To apply for the ALTCS program, seniors can visit the Phoenix ALTCS office in person or call (602) 417-6600. 

Other Financial Assistance Programs for Assisted Living

Elderlife Bridge Loan

Elderlife Financial Services offers bridge loans for Goodyear senior citizens who require assistance covering the cost of a move to assisted living. These loans offer flexible terms of up to 24 months, which can provide enough time to sell a home or other assets or for approved benefits to begin.

Up to six applicants are allowed per loan application, which can increase approval odds and allow family members to help with monthly payments. Upon approval, funds may be sent directly to the senior’s assisted living facility as automatic monthly payments.

To apply for an Elderlife bridge loan, seniors can complete an online application or call (888) 228-4500.

Veterans Aid and Attendance Pension Benefit

The Aid and Attendance benefit is a cash benefit that’s available to eligible veterans and their spouses who are currently receiving a regular VA pension. Those receiving a VA disability pension aren’t eligible for this benefit. For those who qualify, the pension top-up can help pay for long-term care whether a veteran receives it at home, in an assisted living community or in a nursing home. 

To qualify, an applicant needs to meet one of these criteria:

  • Has poor eyesight in both eyes, with vision of 5/200 or worse 
  • Needs assistance with activities of daily living
  • Confined to bed as the result of chronic illness or disability 
  • Lives with a concentric contraction to five or fewer degrees of their visual field
