I visited this facility

The rooms are small fairly clean. No cable or satellite. They are short staffed and not responsive. Do not bring your loved one here if they need attention from staff. You ring a call button and no response. If family is visiting they are still unavailable. The halls are atrocious and the carpets look like they have not been cleaned or sanitized in years. The website and pictures are deceiving. There is never anyone at the nursing station. It’s a scary place.

I can not say enough about how appreciative I am of the care and compassion Quail Manor staff and the owner Colleen provided my mom for the past 5 years. I have been an ER nurse for 20+ years and have seen the end results of poor care provided to some care home residents. I never had a complaint with the staff at Quail Manor. My mom was well cared for, never had any breakdown of her skin the entire 5 years, never had covid and the staff truly loved and cared about her. If the staff or Colleen felt something was just “off” about my mom or maybe she did not eat well that day, I would receive a call or text that day. The staff stay awake all night checking on the residents, which is huge. Not all resident care homes have that service, instead they have staff sleeping in the care home. My family found comfort and peace knowing she was well cared for.

I lived at Lifestream for 3 years. The apartments are like patio homes with a carport just outside the front door. The residents are nice, very friendly and helpful. The staff was helpful and caring. I moved in during the pandemic. I moved there because my doctors recommended getting help with meals and housekeeping. I am a vegetarian. When i visited there were vegetarian options for meals. They had daily entrees for lunch and dinner and available everyday options. The available everyday menu shrunk during the pandemic and very limited vegetarian options. The meals are included in the rent. Because they did not offer many meals that I could eat i ended up buying my own food and not eating in the dining área. I was paying for food that I could not eat. I asked if could not pay for the meals and was told no they didn’t have meal plans the meals were optional and the rent would be the same. I felt like i was being ripped off. I chose to move out. The food was ok when i visited. The dining area was closed due to the pandemic. When it reopened, the quality was not good. The food is cooked at another LifeStream property and delivered daily to the North Phoenix property. 3 stars are for the caring staff and warm, friendly community. If you are looking for a safe, friendly community then this might work for you. I moved out 1 year ago.

this place has frequent bed bug infestations. management is rude to residents. you'd be better off living i a dumptser.

The assortment of floor plans and their square footage compared to other communities. Walk both courtyards and see how beautiful and well kept they are maintained. The staff are very friendly as is the community of residents. A short tour of the facility will verify the close-knit community of residents and their high regards for both the staff and dinning facility. A grateful resident of apartment #159

I am a friend or relative of a current/past resident

Care received was abominable. Staff showed no empathy or compassion. My brother was admitted after hospitalization with several critical health issues. He was diabetic and suffered renal failure. He was evaluated at admission and several troublesome wounds were noted. A care plan was written up, but the wounds were not adequately cleaned and treated. One became so seriously infected that it would have required surgery, had he not passed away before that could happen. Also, in the several days he was there, he never received a bath, despite many pleas from him and from me on his behalf. We were always told it would happen, but it never did. I would definitely not recommend this facility.

I have been a resident of Fellowship Square Historic Mesa for three years now. I am in the independent living, and my wife is in the memory care unit. I like it very well. We started out in a two-bedroom apartment, and it was very nice; it had all the amenities and everything. But when my wife moved into the memory care unit, I moved into a one-bedroom, which is quite nice. I am on the ground floor right by a gazebo, and I can walk down about a hundred feet to the pool where I can swim every day. They have different activities. I had to wait until I was seventy-eight years old before I took up painting, but I really am enjoying that. They have other activities like bingo, card games, and stuff like that. I am only probably a football fields length from where my wife is. Earlier today we both just finished our second shots of the COVID vaccine, and now I can start walking over and spending time with her. The staff is very good, but because this is a non-profit organization, the pay is not as high as it should be. A lot of the young people start out here but then after they get good, they move on. But the staff we do have is like part of our family. Because of the virus right now we are getting our meals delivered to us, which reminds me of another thing. I get rent, utilities, TV, and food all for one price. It is quite nice. There is one à la carte that I do, and that is my personal laundry. We have laundromats all over the place, but for $5 they will do my laundry, which is a large basketful, and it is worth it to me to pay that. That is the only extra that I pay. Communication with the administration is the thing my family lacks the most. They do not communicate, although we do have a brand-new manager, and she has been sending out weekly flyers. So it is improving greatly. We have a resident food committee who meets once a month, and each building has a building captain. They receive complaints and suggestions for improvement from all the residents and meet once a month. They discuss some and put it in the publication every month. Because of the COVID virus things sort of got bad, but our dining room is opening back up this coming Monday, and it is like Christmas for most of us. That is a big social hour, and we all look forward to it. You have your groups you eat with and everything and pick up all the gossip. There are some activities I wish they had but they don't because this is a non-profit. I knew that when I came here, because I researched about six or seven different facilities, and a lot of them had what I wanted but the price was too high. I settled for price over activities because I still drive, and I can do some of my own stuff. Considering what I get, it was a good thousand dollars less than most of the competition. It is a non-profit, so you have to look at those things; that is all part of the makeup.

My friend has been a resident at Ana and David Assisted Living for over two years. As with all of us, my friend wanted to be in her own home and care for herself. But she realized that, after a number of falls and several trips to the hospital, she was unable to care for herself any longer. The staff has been caring and loving since the first day. They understand the physical and mental needs of the residents, and treat the residents as if they are their own family members. My friend comments frequently how much she likes the food, which are home cooked meals each day. With the impact of COVID-19, the staff restricted visits from family and friends. This was very difficult for my friend, who is a very social person. She didn\'t understand the full impact of the coronavirus and why no one came to visit with her. As time passed, I was able to visit and saw that the care and concern of the staff was exemplary. Their major concern was for the health and welfare of the residents. I am grateful that my friend\'s family found Ana and David Assisted Living to give her that care and love she needs. This is most important when we aren\'t able to care for our loved one in the latter stage of their life. \'

I am a friend or relative of a current/past resident

My brother is really happy right now. He's been in Madison Meadows for just about a month now. They are really friendly and helpful. His room is perfect for what he needs. It's not really big, but it was right for him. I have no complaints. They fixed everything up before he got in there, and everything was good. I do know that they have activities available, but he hasn't participated in any because of his condition right now. They have plenty of food; sometimes it's a bit too much, actually. They have a pool, an exercise room, a laundromat, and a library.
