College Oak Nursing and Rehabilitation Center is a 120-bed Skilled Nursing Facility located in the North Area of Sacramento specializing in the care and rehabilitation of seniors recovering from a hospital stay. Our goal is to help residents achieve their highest practical level of functioning by combining his or her lifestyle preferences with sound medical treatment. To this end, personalized plans of care are developed by our staff in conjunction with the resident and/or family and with the guidance of the resident's physician. We have been in business since 1962 and routinely achieve very high marks on our annual accreditation survey. College Oak Nursing and Rehabilitation Center is located in the lovely northeast area of Sacramento and boasts close proximity to several of the area's major hospitals, bus lines, and freeways
