Colorado’s Medicaid program is called Health First Colorado and provides low-cost or free health care services to qualified individuals across the state.
Benefits of Health First Colorado include:
- Doctors, urgent care, emergency room and hospital visits
- Transportation to and from health care appointments
- Behavioral health care
- Prescription drugs and laboratory services
- Speech, occupational and physical therapies
- Preventative care and wellness services
Individuals and married couples aged 65 and over who are interested in enrolling in Health First Colorado must have a
monthly income of no more than $771 for an individual and $1,157 per couple, and assets must be less than $2,000 for an individual and $3,000 per couple.

online application, call (800) 221-3943 or visit the Broomfield County Department of Health and Human Services in person at 100 Spader Way, Broomfield, CO 80020.
Broomfield County residents may also complete a
paper application and mail it to the Broomfield Department of Health and Human Services at the address above.
A Colorado Medicaid waiver also exists for people with disabilities and seniors over age 65 who wish to live in an assisted living home.
Elderly, Blind & Disabled Waiver
Senior Medicaid recipients in Broomfield may be able to receive services in an alternative care facility through the
Elderly, Blind and Disabled waiver. The EBD waiver is designed to provide assistance to seniors aged 65 and older who are blind or have a functional impairment. Those under age 65 may also qualify if they have HIV, AIDS or a disability that requires caregiver assistance to live in the community.
Additionally, individuals interested in enrolling must meet certain income and asset criteria:
- Earn a maximum of three times the monthly federal SSI limit ($2,313 for an individual and $4,626 for a married couple)
- Have less than $2,000 (individual) or $3,000 (married couple) in countable assets
EBD recipients receive Health First Colorado benefits and also have access to the following services:
- Personal care services in an assisted living facility
- Adult day health services
- Respite care
- Medication assistance
- Transportation services
- Home modifications and homemaker services
- Home delivered meals
- Transition services
To apply for the EBD waiver, those already enrolled in Health First Colorado should contact their Broomfield Single Entry Point Agency, Adult Care Management, Inc. in Lafayette, at (303) 439-7011. Those who aren’t yet enrolled in Health First Colorado should
apply.
Note: The EBD waiver isn’t an entitlement program, those who are eligible may be placed on a waiting list to receive services.
