Service Options Using Resources in a Community Environment combines primary care, social services and long-term care programs for seniors who live at home or in assisted living communities.  

The types of services provided may include:

• Skilled nursing care

• Respite care

• Personal support services

To be eligible for SOURCE, an individual must be 65 or older with a chronic medical condition or disabled, determined to need nursing home level of care and eligible for full Medicaid.

Home and Community-Based Services

Home and Community-Based Services (HCBS) help seniors aged 60 and older live independently. Seniors must demonstrate the need for care that’s available in an institutional setting but be able to live safely within the community.
