West Anaheim Extended Care is a skilled nursing and rehabilitation center that provides a full range of services to meet each individual's personal goals and needs. The dedicated team of professionals is committed to meeting the needs of residents in a nurturing environment. The highly skilled nurses provide the best quality care for residents requiring IV therapy, dementia care, wound care, respite stays, enteral feeding program, catheter care, colostomy care and ileostomy care. They also offer hospice and restorative nursing services. The rehabilitation team is committed to creating individualized therapy programs based on personal goals with physical therapy, occupational therapy and speech therapy offered as part of the program. Outpatient clients receive treatment from the same therapists throughout the entire program with an emphasis on maximizing independence and ability. 

West Anaheim Extended Care seeks to maximize quality of life for long-term stay residents by developing individualized treatment plans in areas such as ambulation, range of motion, activities of daily living, positioning, alignment, skin integrity, wound management, bowel training and bladder training, with rehabilitative dining offered as well as a full-time registered dietitian available. Professional staff that specializes in games and outings are employed to create social programs that match resident capabilities.
