Basic services include medication assistance, medical appointment scheduling, assistance with bathing, housekeeping and laundry services. Transportation services are available at additional costs. The staff updates families and physicians regarding residents' progress through regular communication and reports.
Daily activities at Choice Care Senior Home include arts and crafts, board games, reading, gardening and music therapy. Well-balanced meals are served daily, and salon services are available at extra costs. We have trained staff on-site at all times. Our care is intimate and highly individualized to address the personalized needs of each of our residents. We provide three nutritional-based meals with snacks in between, laundry services, housekeeping, and assistance with bathing, grooming, eating, toileting, and ambulating as needed, and medication assistance.

We believe poor oral health is a risk factor for serious systemic health problems in the elderly. Nutrition also suffers with decreased dental health. Every day your loved one will have the attention they need to maintain good oral health.
Services and Fees
At Polly&#39;s Place your loved one will be cared for by trained, loving and devoted staff. It is our commitment to give each individual the care they need. We meet with each resident and family member prior to move-in to ensure everything is done to make Polly&#39;s Place feel like home. We provide everything your loved one needs, when they need it. Some of our services include:
Personal assistance with daily needs like bathing, dressing, personal hygiene and medication management. 
 Assistance with feeding (we will prepare any food)	
 Nutritious meals, snacks, and beverages (served all day and night) 
 Phone/Cable TV/Internet services 
 Planned recreational, religious, social and leisure activities 
 Personal care items (deodorant, mouthwash, toothpaste, incontinence supplies, etc.)

Residents of Polly&#39;s Place will incur a monthly cost of $4,200.00 and can be confident that there will be no extra charges at the end of the month. We are currently a Private Pay home and cannot accept insurance as a form of payment. We give a discount for using direct deposit and can accept most credit cards. Residents are responsible for medical expenses and cost of prescriptions.
