<p>Heaven House Elmwood is a residential care facility that provides assistance with daily tasks, such as getting dressed or putting on shoes. A registered nurse visits weekly and is on call for wellness checks and to deliver a physician-guided, individualized care plan. The staff physician makes monthly visits, and residents have the option to become patients. </p>
<p>This single-story home blends privacy with social opportunities in a small, family-like setting that houses no more than five residents at a time. Meals are served in a communal dining room three times daily, along with numerous snacks.
