Located on Eldridge Parkway near Ray Miller Park in Energy Corridor, Autumn Leaves of West Houston is committed to serving those with Alzheimer's and other types of dementia. Everything we do is by design, from the physical construction of our building to our care plans and welcoming open-door policy. As an example, our offices are within the residence and there are no locked doors, allowing residents to come in and out as they would in their home.

Available Services

We are dedicated to delivering around-the-clock dementia care. Residents are first assessed by our skilled memory care nurse, who creates an individualized care plan and coordinates all services. Each staff member receives extensive specialized training on how to best assist and respond to those afflicted with Alzheimer's and dementia.

From there we strive to support their independence, creating meaningful interactions throughout every day with our Life Engagement Program. Autumn Leaves of West Houston residents partake in a variety of activities, from physical exercise to spiritual pursuits like meditation and song. Additionally, your loved one will enjoy three wholesome meals daily developed by our in-house dietitian and chefs, using only the freshest ingredients.
