Believe every horrible review you see. They are chronically understaffed and your family member will not be treated well. We placed our mom here for short-term rehab and we cut the visit short due to her constant and chronic neglect. We asked all the hard questions and went on the tour and they will do the song and dance about how great things are and then you realize it is all about the money and filling a bed. LuAnn was my mother’s care manager and she was inept; that is being kind. My mom is a stroke survivor completely paralyzed on her right side, incontinent, fed through a peg tube and non-verbal. We had extensive conversations with the front office staff about her needs BEFORE she arrived. We were assured they had the expertise and ability to handle all of her needs. We should have known there was a problem when we had to provide her feeding pump. They had an influx of patients that needed them and their others were out for repair. Ok, no problem; we brought hers. It wasn’t until I reminded LuAnn that she couldn’t bill Medicare for the supplies that we were providing that she seemed to be more proactive about the supply issue. We protected my mom from skin break down and bedsores during our tenure of care so we took them the things we needed to ensure she had the same level of care there. She did not. Several times we came in to find mom soaked in urine or covered in her own feces. To make my accounts truthful, we made sure to photograph my mom upon arrival and then at night when we left. My mom was bathed once in the over 2 weeks she was there and that was at my insistence. My sister is an RN and at one point we were so frustrated with some of the staff and their incompetence that she and I began to change, clean and redress mom. She had a huge bruise on her arm from someone either aggressively holding her or trying to restrain her while they did their “weekly” labs. They don’t really have any lab results to show you but it is a cost they can they can send to Medicare. I could go on but the bottom line is DO NOT PLACE anyone you love here if you cannot do daily check-ins. I was able to observe a few people calling out for help down empty corridors when staff had to work other areas. They use the nurses to pass trays and other duties so you will need to wait if you need your nurse. I observed the same woman fall 3x’s and she was there for rehab. She had a cast on one arm. The first time I came upon her she was outside my mom’s door screaming for help down an empty hallway. I ran to get staff. The second time she had fallen out of the bed and it took staff about 5 minutes to find her. One gentleman screamed for over an hour for an aide or nurse: I timed it. I questioned the dosage of a narco my mom was still displaying signs of pain. Long story short, my peppering LuAnn with continuous questions upset her so much she screamed “I can’t be in here either you all day answering these questions, I have other things to get done.” This after saying my mom was one of her “Angel” patients and we should bring all of our questions to her. The final straw was walking in to find my mom had been left wet overnight. We arranged transport the same day and discharged the next day. My sister and I have taken care of our mom since she was released from ICU. Because of Covid and everything shutting down, we were too afraid we would never see her again. So my overprotectness was because my mom can’t speak yet and I am her voice and advocate. This facility is about dollars. It is not about critical care. They will lie and say you can monitor with cameras in the room and then have continuous wi-fi problems so you won’t be connected with a good signal or any signal at all. Avoid this place at all costs.

My Father was transferred here after breaking his leg. It was the most horrible experience I’ve ever had with a facility, and with a mother who battled cancer for 20 years... there have been a lot. They did not communicate with me for 35 days. No call backs, no contact at all. Only after I demanded to speak with the administrator did his care manager call me back. He arrived with a small cut on his ankle that they let fester until it was the size of a golf ball hole. He was there for 2 months before they even started him on antibiotics! He went a month without being able to use his CPAP, even though they list themselves as CPAP and BYPAP therapy providers. Worse even, he would have to take it off during the night even after they provided him with the necessary water, because it would run dry. They would then lie to me and tell me he was refusing to use it. My father used it FAITHFULLY for 6 years prior! When he heard them lying he refuted what they said, and his terrible care planner Luanne just changed the subject. They ignored his specialists instructions to the point the doctor became angry. Incident after incident occurred and they would lie about it, but then order x-rays to make sure he wasn’t hurt....after “nothing” happened. My father actually regressed in their care. I made plans to transfer him and they blocked the other facility from speaking to him and refused to return calls to facilitate a visit. When the other facility used a nurse thru FaceTime to meet with my dad and they found out he would NOT be staying as a resident, they moved to discharge him within 13mins. Knowing he was not yet ready, that would of meant discharge to the street!! THIS FACILITY IS HORRIBLE!! I would NOT BOARD MY DOG HERE.

Walnut Creek is very pretty and well-kept. Everyone's been very nice and accommodating. My mom is not really pleased with the food but she's picky. They have arts and crafts, and there's usually something going on every day, but nothing at night. Her room is tiny but fine.

We liked it. We told them that if a 1-bedroom becomes available that they should call me. They had no 1-bedroom available; they only had efficiency. The staff members were very nice. Everything about the place was nice. I saw people together playing puzzles. One thing I did like about them is that they offer to take residents to different places twice a week. I thought that was awesome. They have a library too. They offer six meals. If they had a 1-bedroom, I would have gone there.

Isle at Watercrest - Mansfield was lovely. I would love to put Mom there. The staff was well informed and very courteous. The decor was outstanding and updated. They had live plants and better air.

There is nothing about Isle at Watercrest – Mansfield that makes you think it is a nursing home. There isn't a medical look to it. It looks like a hotel. Everyone is wonderful, nice, and friendly. The grounds are excellent. My mother's one-room is very nice, clean, well maintained, and accommodates her needs. They have bingo, cards, movies, and a live entertainment schedule.

Isle at Watercrest is assisted living, but the care level was more than what my mother needed, so it was not a good fit. It looked brand new, it was beautiful, and it was very nice. The staff I met with was very nice.

Brookdale Mansfield was a little more expensive because you have more assistance. The rooms were smaller and the price was higher. The staff did fine.

Brookdale Mansfield is beautiful, nice, very clean, and smells good. The residents seem happy. They have little group settings where they talk, chat, and watch TV, so it seems very good for the two days we have had my sister there. The dining area appears to be very good. My sister has dementia and said the food is very good.
