Wellington Meadows is an assisted living community that takes pride in providing a second home to active seniors and encourages residents family also be part of our family. Our aim is to provide seniors quality lifestyle, home and health care to enhance the lives of our residents.

Wellington Meadows is a 28-bed CBRF facility located on five acres of land in the city of Fort Atkinson, Wisconsin. Wellington Meadows is located adjacent to the Medical Center. We are pleased to offer an atmosphere where residents and families feel comfortable and at home. Come by and visit the facility and its beautiful indoor and outdoor areas.

Featured Amenities

Our community have an array of floor plans that residents can choose from. We have 24 homes that are all beautifully appointed and spacious. We have studio apartments and companion suites ready for occupancy. All homes has its own private bath, a large window with a beautiful outside view and emergency call systems for resident’s safety.

Wellington Meadows has a lot of amenities to offer residents. We have a beauty salon and barbershop where residents can get full services. If residents want a quiet time and curl up to read a book we have a library stocked with good reads and a cozy living room with fireplace.

Available Services

Residents of our community receives customized care program and are assisted with competent and experienced staff that are available onsite 24 hours a day. To keep our residents live life comfortably we provide assistance in eating, bathing, dressing, grooming, toileting, mobility, and medication management.

Delicious meals are served fresh three times a day with snack available always. Activities are prepared daily to provide enrichment, stimulation, and keep them active. We also provide transportation services if residents need to go shopping or to a doctor’s appointment.
