Description of Van Duyn Center for Rehabilitation and Nursing
Van Duyn is a certified residential skilled nursing facility, providing short-term rehabilitation and long-term care to individuals with health conditions that require higher levels of medical and nursing services.
Under the supervision of Van Duyn's medical director, a team of physicians cares for each resident. A doctor is assigned to each nursing unit to assure continuity of care. Consultant physicians in the areas of dermatology, podiatry, urology, ophthalmology, neurology, dentistry, surgical and ENT are available to this qualified medical staff. Outpatient lab and x-ray services are provided, as needed.
Registered professional nurses, assisted by well-trained and qualified licensed practical nurses and certified nursing assistants, provide 24-hour care. Nurse practitioners are also on-site each day to support the nursing and medical teams.

I went to Van Duyn for rehab because I was having trouble walking. Therapy encouraged me to walk again, actually all the staff did. The concierge team was kind and helpful.

I would recommend Van Duyn, this was absolutely the right choice. Nursing was fantastic; therapy was phenomenal; recreation was excellent and the concierge didn't disappoint. I would return without hesitation.

I didn't use all of the services during my time, but the Physical and Occupational Therapy was a good service. All of the services were good, and the staff made my stay very comfortable.

I feel like I made the right choice by coming to VD! Concierge was great, social work was great, housekeeping was good, and nutrition was great. P.T.- that gym was off the meter! My therapist pushed me to excellence. My respect and appreciation for them I could never fully express.

I would recommend Van Duyn! The LPN Liza, and the RN Candice made my stay more comfortable than I could have imagined. The nursing, housekeeping, therapy, and social work teams were all great! The concierge Bernadette even prayed with me.

I would recommend Van Duyn, I liked the service I received from the Nurse Practitioner and the Podiatrist. I didn\'t realize the service was provided there. Megan the Director of Special Work deserves 5 stars! The Nutrition and Housekeeping Team were A+

I would recommend Van Duyn, Melissa and the 5th floor staff were great! All the Nursing staff were patient and kind. Therapy was outstanding. My brother received the most loving care from everyone he encountered at Van Duyn.

I had a very positive experience at Van Duyn Center for Rehabilitation and Nursing. Tarana, CeCe , and Keisha were great. The occupational therapy and physical therapy teams were awesome for helping in my recovery. I had spectacular wound care from Pam, and the respiratory was good as well.

I liked the physical therapy a lot, it improved my mobility. Bella from social work was good and had good communication skills. Good, housekeeping, laundry were all good. Business office was polite. Concierge tended to my concerns and brought my thing my daughter dropped off.

I could not have been happier with my stay at Van Duyn Center for Nursing and Rehabilitation. The food has gotten better. The therapy was phenomenal (with special thanks to Chris, Yousef and Kurt) and I even enjoyed the recreation. Overall, I was pleased with this caring facility and my experience was positive.

Chris and Carol in Physical Therapy were great and not too pushy. The Nutrition team was great, and they asked about my dietary preferences. Their Housekeeping was an absolute sweetheart. It was nice to go on the patio and the Concierge was very helpful. Van Duyn Center accommodated me very well.

I met a lot of nice people at Van Duyn Center for Rehabilitation and Nursing. I liked Maria, she\'s a good nurse, and the aide, Jeneene is great. I thought Kearah Lee was also a great nurse. Physical Therapy was great. Brittany and Angie really pushed me, which is what I needed.
