Brookdale Nacogdoches (formerly Clare Bridge of San Antonio) is a caring community, providing compassionate assistance to seniors experiencing Alzheimer's and other forms of memory loss. Specifically designed for those who have experienced memory loss to better navigate, we provide large, circular paths and way-finding cues. Community interior spaces are designed to re-create environments people have enjoyed throughout their lives, which are scaled and decorated like those of a private home.

Enclosed outdoor walking areas give residents the ability to explore their surroundings in complete safety. Our fully carpeted studio and one-bedroom apartments are cozy and bright, and to better identify personal areas, we recommend that residents decorate their spaces with personal furnishings and cherished mementos. Small pets are also welcome in our friendly community.

Available Services

The Clare Bridge Daily Path provides a gentle structure targeting the specific needs of those with Alzheimer's and other forms of dementia. Residents participate in mental discussions, brain stimulating games and reminiscing. Physical programs include walking and dancing. Individual or group activities provide a sense of purpose, meaning and belonging.

We offer a dining program designed to help residents successfully enjoy the dining experience, providing dementia-friendly menus, individualized assistance and consistent seating arrangements. Nursing staff are on call 24 hours a day, and special services include incontinence management, ambulation and escort services.
