My mom went to this facility for rehab in October 2022. The treatment there was horrible. The first week she developed an allergy to her antibiotic and it took me 4 days of reaching out to the staff for someone to do something about it. The following week she fell and went to the hospital. A week and a half after that my mom started to de-compensate, I told them that something was wrong with my mom and they didn’t do anything about it. On the third day after chasing nurses and telling them that my mom was not doing well, I administered a home test to check for urinary tract infection. She tested positive, i immediately told the nurse and she told me to wait two minutes, two hours later I was informed that she had left, I told the nurse that was there and he did nothing. The following day I found my mom in a wheel chair alone in the dining room with vomit all over her cloths her dentures were on the floor and she was unresponsive. Immediately I told the same nurse from the previous day and all she did was touch her forehead and told me that she didn’t have a fever and that she was going to give her an IV. I insisted I wanted to take my mom to the hospital. After some back and forth my mom was taken to the hospital. By the time my mom got to the hospital she had 104 fever and they found she had a urinary tract infection and acute kidney lesion. She was hospitalized for 10 days. The staff here is heartless the signs that my mom was not doing well were obviously and no one did anything.

This facility keeps patients against their will. Some staff members are true angels but my grandfather has been asking to check himself out for at least a week and he is still there even though we have home care in place. They will not provide a discharge date so we can have our live-in aide ready to go so no matter what we will have to scramble. The food is cold and inedible. They promise a shower but it keeps getting put off. Thank god we visit daily because I can’t imagine how much worse this would be.

My mother has been at this facility for a couple months now, and I can honestly say it was the best decision ever. From the 1st day of the tour, the administrator made me feel so welcomed. It felt at home, and to top it off my mom made friends during the tour. My mother ended up staying for lunch and didn’t want to leave. The room we have is spacious with its own bathroom, the staff has been so cooperative and communicating with me constantly since i have been away. I will definitely recommend this facility to anyone.

HORRIBLE. Staff is rude and lazy. Supervisor’s are useless and social workers are rude and arrogant. The only person who ever seemed to care was The concierges. Would NEVER recommend this place for an animal let alone a human being.

The staff is 10/10 amazing they make my mom feel right at home. One of my biggest fears was my mother acclimating to a new environment and she fit right in everyone including residents made her feel so at home and I couldn\'t be more grateful Thank you Rosewood truly a blessing.

I visited Maple Pointe for my father, and that was very nice. It was very clean and inviting. The staff were very friendly and caring. They have a little bingo and some card games. They showed me the outdoor area, the dining area, a movie theater, and a library.

I currently have Home Instead Merrick helping with my husband's care. They help out with showering and preparing lunch or breakfast, whichever is needed -- but it's mainly lunch. They see that he's dressed properly because he puts on daytime clothes at night and nighttime clothes in the daytime. They check after him and clean up if needed. They also do light housework, vacuuming, and take him for a walk when they can. Today he has a dental appointment, and the caregiver will be accompanying us. It's his first visit to that dentist so I'm going, but the caregiver will be driving us and acting as a companion to help him understand what's going on. They do whatever I ask them to; sometimes it's an errand, and sometimes just getting lunch for him if I don't have anything prepared. It's whatever I need at the time, like washing clothes and folding them from the dryer, and things like that. Whenever I have to speak with the manager or the people at the office, they're always very kind, very thorough, understanding, and easily reachable, so I've had no problem whatsoever. I've only been with them maybe three months, but they have been very responsive. If somebody could not come in, which happened a couple of times, they would let me know ahead of time and determine with me whether or not I wanted a replacement. I have not been with them that long, but I'm very pleased with them.

I really liked Somerset Gardens Senior Living. It was actually on the top of our list. They had a ton of parking, and even though it was older, it was still homey. It was actually a converted hotel. I really like that most of their rooms have outdoor balconies, too. The staff was very, very accommodating, pleasant, and excellent. The halls were a little narrow, though. I think they should have been a little wider, but that was fine. It was clean and had no smell.

I had my dad in this facility but removed him due multiple hospitalization from falls and urinary tract infections that became septic. The nursing staff is not qualified to oversee any type of medical care. No recreation to speak off. Housekeeping mops the floors with dirty water and then sits around watching tv all day..

Senior care costs vary throughout the country, and Genworth’s 2018 Cost of Care study has collected cost data from a number of cities and counties, making cost navigation a little easier. The study shows that at $4,425 per month, assisted living in Nassau County costs more than the state ($3,500) and national ($4,000) averages. Costs are similar in Tallahassee ($4,339), while with the exception of The Villages ($5,031), assisted living prices are lower in nearby cities, including Gainesville ($3,775), Ocala ($3,275) and Deltona ($3,850).

Care Cost Comparison
As with all other areas, Nassau County has a wide range of costs for senior care. Nursing home care in a state-licensed facility is the most expensive option, costing close to double assisted living costs. On the other end of the price spectrum is adult day health services; however, it’s important to note that these services are provided in a community-based group setting on weekdays only. Assisted living costs $135 more per month than a home health aide and $775 more than a homemaker based on 44 hours of service weekly. While the up-front cost of assisted living is slightly higher than in-home senior care, assisted living includes room and board. This means the total monthly expenses may be lower for seniors who choose to join an assisted living community rather than remain in their homes.

Financial Assistance for Assisted Living in Nassau County
Medicaid in Florida
Millions of Americans who have low income and few assets and who would otherwise not be able to afford health insurance are covered under Medicaid, the nationwide health protection plan. Funded by the state and federal governments and administered by each state, Medicaid provides basic health care, emergency care and long-term services for eligible seniors.
To qualify for the Florida Medicaid program, seniors can earn no more than three times the federal poverty level, currently set at $2,313 a month ($4,626 for couples). Medicaid applicants can also own no more than $2,000 ($3,000 for couples) in countable assets, such as cash, investments and real estate, although the applicants’ home is exempt as long as it’s valued at $585,000 or less.
Note: Seniors who receive Social Security Income payments automatically qualify for Medicaid, and no further information is needed unless they wish to apply for long-term care through Medicaid.
Also note that since Florida is an income cap state, seniors cannot spend down excess income to achieve Medicaid eligibility.
Statewide Medicaid Managed Care Long-Term Care Program
Seniors enrolled in Medicaid who require the level of care normally provided within a nursing home and who wish to remain in an assisted living residence may be eligible for long-term care services through the Statewide Medicaid Managed Care, Long-Term Care Program.
The SMMC LTC is a Medicaid waiver program that prevents or delays nursing home placement among seniors by providing medically necessary supports delivered within an assisted living facility.
To qualify, seniors must complete screening through the Comprehensive Assessment and Review for Long-Term Care Services (CARES) program, which assesses the long-term care needs of each senior. This assessment is also used to determine which services seniors enrolled in the SMMC LTC receive.
If approved for this Medicaid waiver, seniors may be provided with skilled nursing care, medication management, personal care, homemaker services and other supports. Seniors are also assigned a case manager to review their care on a regular basis.

Other Financial Assistance Programs for Assisted Living
Optional State Supplementation
Supplemental Security Income recipients who are 65 or older and reside in an assisted living facility because they’re unable to live on their own may qualify for a monthly SSI cash top-up through the Florida Optional State Supplementation program.
Seniors may be eligible for up to $78.40 per month ($156.80 per couple) under the OSS. These state-provided funds are added to the monthly SSI payment and intended for use towards the room and board portion of assisted living costs.

The Nassau County Council on Aging operates two Senior Life Centers in Hilliard and Fernandina Beach where seniors can participate in exercise groups led by experts from the YMCA, join in a sing-along, play a game of bingo and enjoy escorted day trips.
